Weird, it's a new form thing. (Google Forms upgraded, I checked an option that emails you a "receipt" so you can keep track of your @#$%&.)

I will either try to figure it out, or get annoyed and leave you with the burden of double filling it out. ;-)

When you create forms, somewhere there is a checkbox to have it collect emails, which adds that field at the top, in addition to the field you created for that.
